65 years old female patient who is from nalgonda labor by occupation presented to OPD on 12-01-23
CHEIF COMPLAINTS:
Stomach pain since 2days
Associated with nausea and vomiting since 1day
HISTORY OF PRESENT ILLNESS:
Patient was apparently asymptomatic 2 years back then she developed pedal edema, facial puffiness, decreased urine output short ness of breath and difficulty in moving lower limbs and was taken to a private hospital and diagnosed to have hypokalemic (k+2.2) and found to have raised creatinine levels and was started on conservative management
1 year back patient started walking with support and pedal edema and facial puffiness and diagnosed with decreased urine output and diagnosed with CKD ( grade 3 RPD changes, increased creatinine, shrunken liver,anemia) and started with conservative management
2months back pain abdomen, decreased appetite, burning micturition and cloudy urine for 6 days subsided on medication.
2days back pain abdomen - squeezing type not associated with loose stools
Vomiting-assosiated with nausea and pain abdomen Non bilious ,non projectile, 2 episodes, food particles as consents .
PAST HISTORY:
N/K/C/O DM, TB , EPILEPSY,ASTHMA.
No history of past surgery
FAMILY HISTORY:
No similar complaints in family
PERSONAL HISTORY:
APPETITE : Normal
DIET: mixed
SLEEP : disturbed
BOWEL AND BLADDER : regular
MICTURITION : decreased
Addictions: Alcohol occasionally (stopped 10 years back), zarda chewing daily 3 times but stopped a year back
GENERAL EXAMINATION:
Patient is conscious coherent and cooperative, well oriented to time,place and person.
Thin built and moderately nourished
Pallor is present.
No signs of Icterus cyanosis, clubbing , Lymphadenopathy.

Temp:afebrile
PR: 90 bpm
Bp: 120/70 mmHg
RR: 20 cpm
Systemic Examination:

No Thrills,
S1,S2 sounds heard,
No murmurs.
RESPIRATORY SYSTEM:

No drooping of shoulders
No supraclavicular hollowing
No kyphoscoliosis
No use of accessory muscles for respiration

No intercoastal widening or narrowing
Chest movement: symmetrical
Measurement of chest expansion

Hemi Thorax:17cm

Vesicular breath sounds
No wheeze
ABDOMEN:

No Local rise of temperature
No palpable Mass
Spleen and liver not palpable
